Backflow Testing in Ohio: What It Is, Who Needs It, and What the Test Involves

If you own a commercial building, a restaurant, a rental property, or even just a lawn irrigation system in Northeast Ohio, there is a good chance your water supplier requires you to have a backflow assembly tested once a year — and to file the paperwork proving it. Most people first learn this when a notice arrives in the mail with a deadline on it.
This guide explains what backflow actually is, why the annual test exists, who it applies to, what the tester does when they show up, and what happens if your device does not pass. It is written for the people who receive those notices, not for plumbers.
What backflow actually is
Water in your building is supposed to travel one way: from the public main, through your meter, out your fixtures. Backflow is when that reverses and water from your side of the meter gets pushed or pulled back into the clean supply.
It happens for two reasons. Back-pressure is when the pressure inside your system becomes higher than the pressure in the main — a boiler, a pump, or a pressurized tank can do this. Back-siphonage is the opposite: pressure in the main drops suddenly, usually because a water main broke or the fire department opened a hydrant nearby, and the resulting suction pulls water backward out of buildings.
On its own, reversed water is not the problem. The problem is what it can carry with it. Any point where the drinking water supply can touch something else is called a cross-connection — a hose left sitting in a mop bucket, an irrigation line running through fertilized soil, a boiler loop full of treatment chemicals, a soda machine's carbonation line. Under normal conditions none of that moves. Under a pressure reversal it can travel into the pipes that feed drinking fountains.
Who is required to test in Ohio
Requirements are set and enforced by your local water supplier, so the exact wording varies between Warren, Youngstown, Niles and every other system in the Valley. In practice, if any of the following describe your property, you almost certainly have a testable assembly:
- Commercial and industrial buildings — nearly all of them, particularly anything with a boiler, chiller, or process equipment connected to the water supply
- Restaurants and food service — carbonation systems, dish machines, and grease-handling equipment all create cross-connections
- Buildings with fire suppression systems — sprinkler loops hold standing water, sometimes with antifreeze in it
- Lawn irrigation systems — including residential ones, which surprises a lot of homeowners
- Medical, dental and laboratory facilities — usually the strictest requirements of any category
- Multi-unit residential buildings — depending on how the system is configured
The near-universal cadence is once every twelve months, plus a fresh test any time an assembly is installed, repaired, relocated, or replaced. Miss the deadline and suppliers can and do issue violation notices; continued non-compliance can escalate to a shutoff.
What the test actually involves
The visit is shorter than most people expect — typically well under an hour for a single assembly, assuming nothing is wrong.
The tester attaches a calibrated differential pressure gauge to the test cocks on your assembly and closes valves in a set sequence to isolate each internal component. What they are checking is whether the check valves still seal under pressure and whether the relief valve opens at the correct point. Springs weaken, rubber seats harden, and debris gets caught in the seal — all of which let a device fail quietly while still passing water normally. You would have no way of knowing from the outside, which is the entire reason the test is scheduled rather than complaint-driven.
Your water is off for the few minutes the assembly is isolated, so it is worth scheduling around business hours if a shutdown is disruptive.

Afterward the tester completes a report with the gauge readings, the device serial number, and a pass or fail result. That report has to be filed with your water supplier — the test alone does not make you compliant. Ask whoever performs your test whether they file on your behalf or hand you the form to submit yourself; both are common, and assuming the wrong one is how properties end up with a violation despite having passed.
The devices themselves
Which assembly you have depends on how severe the consequences would be if backflow occurred at your property:
- Double check valve assembly (DCVA) — two independent check valves in series. Used where a cross-connection would be objectionable but not a health hazard.
- Reduced pressure zone assembly (RPZ) — two checks plus a relief valve that dumps to atmosphere if either fails. Required for high-hazard connections. If you see one discharging water onto the floor, that is the device doing its job and telling you it needs service.
- Pressure vacuum breaker (PVB) — common on irrigation systems, protects against back-siphonage only.
- Air gap — a physical vertical gap between an outlet and a receiving fixture. No moving parts, nothing to test, and the most reliable protection there is where the layout allows it.
If your assembly fails
A failure is not unusual and it is not a crisis. Assemblies are mechanical, they sit under constant pressure, and parts wear.
Most failures are repaired on site with a rebuild kit — replacing the seats, springs and rubber components inside the existing body. The assembly is then retested immediately and, if it passes, the passing report is what gets filed. Full replacement is only necessary when the body itself is cracked or corroded, or the device is obsolete enough that parts are no longer made. Either way the repair and retest usually happen in one visit, which matters when a supplier deadline is close.
Keeping the system healthy between tests
Annual testing catches failures after the fact. A few habits prevent them:
- Do not leave hoses submerged. A hose sitting in a bucket, a pool, or a floor sink is a live cross-connection for as long as it stays there. This is the single most common one we find, and it costs nothing to fix.
- Watch for discharge. An RPZ that periodically dumps water is signalling a fouled check. Occasional discharge during pressure fluctuation is normal; continuous discharge is not.
- Protect the assembly from freezing. Outdoor and irrigation assemblies split in a Northeast Ohio winter if they are not drained or insulated, and a cracked body means replacement rather than a rebuild.
- Keep your paperwork. Hold onto passing reports. When a property changes hands or a supplier audits records, being able to produce a history saves considerable time.
- Re-test after any plumbing change. Adding equipment can create a cross-connection that did not exist at your last inspection.
Choosing who does the testing
Not every plumber can perform this work. Backflow testing requires a separate certification and a gauge calibrated on a documented schedule — an uncalibrated gauge invalidates the result regardless of who read it. Before booking, it is fair to ask three things: whether they hold a current backflow certification, when their gauge was last calibrated, and whether they can repair a failed assembly on the same visit rather than scheduling a return trip.
That last question matters more than it sounds. A tester who cannot repair leaves you to find a plumber, schedule the fix, and then book a retest — three appointments and often a missed deadline, instead of one visit.
